Coordination of Benefits
Clients are responsible for understanding the details of their insurance benefits, including mental health coverage, co-pays, deductibles, and any limitations on the number or type of covered sessions.
It is the client’s responsibility to verify insurance benefits prior to the first appointment and to inform our office immediately of any changes to coverage, including policy terminations, new insurance plans, or changes in deductibles or co-pays.
If insurance denies coverage or if benefits are exhausted or not active at the time of service, the client is fully responsible for payment of all charges.
Any balances not covered by insurance are the client’s responsibility and are expected to be paid promptly upon notification.
Frequently Asked Questions
Does my insurance limit how many sessions I can have? It’s always best to contact your insurance company to confirm your specific benefits. In general, many insurance plans don’t set a strict limit on the number of therapy sessions you can have. However, they may require documentation of medical necessity for sessions more frequent than once per week. If you’re hoping to attend therapy more than weekly, we recommend checking with your insurance company and discussing your needs with your therapist to understand what frequency of care is covered by your plan.
Why is there a different therapist's name listed on my Explanation of Benefits (EOB) or on my invoice, than the one I am seeing? Some of our therapists are associates who provide therapy under the supervision of a fully licensed therapist. Depending on your insurance plan, claims may need to be submitted under the supervising therapist’s name and license, a process known as “supervisory billing.” This means you may see your therapist’s supervisor listed as the billing provider on your Explanation of Benefits (EOB) or invoice, even though you are receiving care from your regular therapist. Your therapist is still providing your sessions, with support and oversight from their supervisor. Because insurance requirements vary, this billing arrangement is only used with plans that allow it.
What's on a Superbill and how do I submit it to my insurance?A superbill is an itemized receipt for your therapy sessions that includes information your insurance company may need to reimburse you for out-of-network services. It typically includes your therapist’s information, the dates of your sessions, the services provided, diagnosis and billing codes, and the amount you paid.
To submit a superbill, contact your insurance company to confirm your out-of-network benefits and ask how to file a claim for reimbursement. You can then submit your superbill through your insurance company’s online portal or by following their instructions for submitting claims. Reimbursement depends on your specific plan, and receiving a superbill does not guarantee that your insurance company will reimburse you. We recommend confirming your benefits before beginning therapy so you know what to expect.
I have primary and secondary insurance, how does this work?If you have both primary and secondary insurance, please let us know and provide insurance cards for both plans. We will bill your primary insurance first, then submit the claim to your secondary insurance to help cover any remaining costs that are eligible under your plan. Because claims must be processed by both insurance companies, billing may take a little longer.
A common example of secondary insurance is Medicaid (OHP). If you have OHP in addition to another insurance plan, it’s especially important that we have your primary insurance information on file. OHP may not cover services if your primary insurance has not been billed first. If you have questions about how your plans work together, we’re happy to help you understand the process.Engagements range from two weeks for focused audits to several months for comprehensive transformation.
